Community Christian School is a member of the Florida High School Athletic Association (FHSAA).

As a member, FHSAA requires that the same paperwork be completed for every CCS Middle School and High School Athlete. Each athlete must have a current Physical Form (EL2) and the Consent Form (EL3) on file through Final Forms

For homeschooling athletes, additional forms are required. See Final Forms for more information.

Final Forms is the program we are using for our athletic paperwork. The program allows parents to register students, upload forms into the system and roll over the information from year to year. This program ensures paperwork isn’t misplaced and parents can keep the originals. Please do not turn in FHSAA paperwork to the school or the coaches.

All athletes must have the required FHSAA Paperwork submitted prior to a student-athlete being eligible to participate in conditioning, practices or games.

Listed below are the forms that all CCS student athletes and PEP/Homeschool student athletes are required to complete, scan and submit through Final Forms.


1)    Physical form (EL2)

2) Consent forms (EL3) - Electronically submitted through Final Forms.

- PEP Students must complete the EL15 and submit verification of enrollment in the Step Up for Students Portal.

- CEO and Homeschool students must complete the EL7

- Homeschool and PEP Students must also submit the GA4 and an unofficial transcript of classes taken and in progress.

If an athlete has an EL7 on file from last year and played the same sport(s) for the same school, he/she is not required to complete a new EL7.  That athlete must complete an EL7V instead.   However, if an athlete would like to participate in a new or additional sport, a new EL7 is required.  Please contact our Athletic Director Scott LeMay to determine if a new EL7 is required.

If your athlete has changed attendance at a school at any time, regardless of whether the change occurs during the school year or during the summer period between school years, or if your athlete has changed  status to a homeschool student, your athlete must complete a GA4 form.  This form is not required for students entering from a terminating grade school (i.e. 5th grade to 6th, 8th grade to 9th grade).  This form only needs to be done once for each change of schools or change in participation as a “Non-Traditional” student at a member school.

FHSAA paperwork is due before an athlete may begin participation in the sport.

Cost per sport is $175 for CCS and CEO students.  

The fee for PEP and Homeschool students is $285 per sport.
